Hi, I'm Dr. Philip McMillan.

And today I'm going to just be capturing a thought that I had in terms of discussing with a patient some ideas around how you get clinical presentation of disease.

So this is around a real person, but I'm kind of not speaking directly about the person, but extrapolating the situation.

So this is a lady in her mid 50s and she has largely been fit and well but has noticed over a number of years that whenever she has a cold or a viral infection she can sometimes get a little bit wheezy.

This was exacerbated when she was in contact with birds as well.

So it indicates that there was some degree of hypersensitivity that was present that was underlying for the longer term. This lady is vaccinated and she's also boosted with one vaccine and has largely been stable earlier in the year and so I'm speaking to you here in about the middle of 2023 and so about seven months ago she had her first infection with COVID and subsequent to that she was on well for a bit but then settled down and was back to normal.

She's recently had an episode of a significant asthma attack for which she needed to be in hospital to get nebulizers and needed about a week of using inhalers before she got back close to her baseline.
