No testing has been done - as far as I am aware - into the "effects" of injection directly into a blood vessel, which certainly has happened in more than a few cases. All three of these factors may have been in play at once - but we have no information about the specific case.

one interesting thought i just had was related to some recent queries made on the vroman effect as the protein corona gets built.
there is the soft corona, the spaghetti ball that heads for the liver. (and ovaries sigh)
the LNP picks up small fast proteins, then bigger slower proteins come in and shove the smaller proteins off (fibrinogen and 30-40 proteins end of forming the ‘hard protein corona’ underneath the more mutable soft corona. the white clot seeds start in the hard corona. the multiprotein fibrinogen net is there. like piglets vying for teats, the proteins are competing and shoving the ‘runts’ out of the way. they ride the LNP all the way to whatever cell gulps it up.
but… along the way the AI mentioned that most vroman research is on hard surfaces - it said the more pliable LNPs (especially the bigger aggregates) will just hoover everything charged in the blood with affinity over and above brownian motion. the early arrivers in the game of musical chairs don’t actually lose their seat like they would on a hard surface. everybody attractive gets a chair!
i mention this when i remembered your comments of metals in the white clots.
Any kind of charged metal compounds would be grabbed by the LNPs as they go round and round and round the blood whacking RBCs.
